Bowditch, who was dean of Harvard Medical School from 1883 to 1893, suffered from Parkinson's disease, and as a result is supported by the other two men for this photograph. While in Boston, Osler attended an alumni gathering at Harvard Medical School (though he was not an alumnus). He was in Boston as part of a longer North American visit that took him to various cities in the United States and Canada.
